16 years ago today, all 4 crew members lost their lives after Korean Air Cargo Flight KE8509 (HL7451) crashed into Hatfield Forest near Great Halingbury, England. Shortly after taking off from London Stranded Airport to Malpensa Airport, Italy.

The cause of the crash was attitude director indicator (ADI) failure combined with pilot error. The first officers instruments were working & shown the true angle of bank but said and did nothing.

The AAIB issued recommendations to Korean Air to revise its training program and company culture.
